Given the increasing number of patients/consumers with dementia, politcal action is urgently required to prepare the health care services throughout Europe to deliver cost effective high quality long-term care to people concerned. Currently there is a lack of clinical research data of patients/consumers and informal caregivers to develop best practice strategies for long-term care. RightTimePlaceCare intends to deliver best practice strategies for need-tailored dementia care throughout the dementia care sectors and aims to preserve best available health outcomes for both patients/consumers with dementia and their informal caregivers at affordable cost-benefit ratios.
RightTimePlaceCare will describe and analyse the European health, social care and welfare systems, advocacy and informal caregiver support systems for patients/consumers with dementia and intersectorial communication. A European survey will assess the factors influencing the time of admission to institutional long-term nursing care facilities, investigate living conditions and gather clinical data of patients/customers with dementia and their informal caregivers in long-term formal professional home care and institutional nursing care facilities, and the related economic impact. Consecutively best practice strategies will be developed for intersectorial arrangements needed to improve the effectiveness and efficiency of integrated health care in European dementia care systems, and recommendations for best practice models or interventions in long-term care facilities. RightTimePlaceCare will advance the state of the art in health systems research in dementia care and will improve cooperation between researchers to promote integration and excellence of European dementia care research. The knowledge generated by RightTimePlaceCare will empower the policy and decision makers to manage and reform dementia health care systems in view of common challenges and within the common framework of the EU.
Overall strategy and general description
The main objective of the project is improving dementia care health services for European citizens with special focus on development of best practice strategies for the transition from formal professional home care to institutional long-term nursing care facilities. This research will develop the scientific evidence base that supports the Member States to organise their health systems. In order to achieve this objective the work plan consists of six work packages (WPs). Workpackage 1 comprises the overall project management and work package 6 the dissemination of the project. The work packages 2-5 are linked to the specific objectives of the project.
- Work package 1 (Management) aims to coordinate the project.
- In work package 2 (Health Care Structure) we will describe and analyse the European health, social care and welfare systems, advocacy and informal caregiver support systems for patients/consumers with dementia and intersectorial communication using various available sources and in addition focus groups to explore best practice. We will cover the continuum of care from informal care, contribution from the civil society, public home care and the intermediate forms of care to the long-term institutionalised care, including end of life care. In addition, the exploration of the communication between different formal and informal care providers and the experiences of patients/consumers with dementia and their informal caregivers with intersectorial information delivery will be gathered to understand the challenges and to develop an integrated health care model for patient with dementia.
- In work package 3 (Survey) and work package 4 (Economics) we will develop and conduct a survey assessing the factors influencing the institutionalisation of patients/consumers with dementia at the time of admission to long-term institutional nursing care and investigating the living conditions of patients/consumers with dementia and their informal caregivers receiving long-term formal professional home care and institutional nursing care and the related economic impact.
- In work package 5 (Best practice) we will develop best practice strategies for intersectorial arrangements needed to improve the effectiveness and efficiency of integrated health care in European dementia care systems, and recommendations for best practice models or interventions in long-term nursing care facilities.
- In work package 6 (Dissemination) we will develop and implement a detailed dissemination plan aimed to effectively deliver the generated knowledge and best practice strategies to political and other decision makers in dementia care and to ensure a maximum general dissemination of the results.
